The failure investigation typically includes:

• Site visit to inspect failure and condition of machine
• Review of operational parameters
• Review of operation at time of failure
• Calculation of probable loads and stresses in failed components
• Material analysis of failed components
• Supporting finite element analyses, similar to Structural and Mechanical Auditing process
• Report summarizing findings

Failure investigations can be done on behalf of the owner of the machines or on behalf of insurance entities.
